Abstract
Despite the ongoing technological innovation in medicine, care at the patient’s home remains a fundamental component of the treatment process for heart failure (HF) patients. Patient education is a crucial element enabling the patient to fully adhere to treatment and perform effective self-care. Many aspects of HF require the involvement of and discussion between the physician, the nurse, the patient and the patient’s family. Issues to be discussed in detail include limitations imposed by the illness, guidelines for medication taking, self-control parameters, medication side effects, and prognosis. Self-care can only be performed by the patient and their relations if all information provided to the patient is properly received and understood. With the growing incidence of HF, which is one of the leading causes for hospitalization and mortality in Poland, proper self-care is highly advisable. Professional education of the patient and their relations contributes to reduced frequency of re-hospitalizations due to HF decompensation. Properly performed self-care entails a number of tangible benefits, increased patient satisfaction, and increased quality of life. The notion of self-care is gaining recognition and appreciation throughout Europe. A passive role of the patient as a recipient of medical services benefits neither the patient, nor the health care system.References
