Abstract

Chronic heart failure (CHF) presents complex challenges that demand a multidisciplinary approach for effective management the disease. This article investigates into the collaborative efforts of cardiovascular professionals, including cardiologists, cardiac nurses, rehabilitation specialists, dietitians, and psychologists, in delivering comprehensive care for CHF patients. Drawing upon European and American guidelines, it explores the integration of patient-centered care, contemporary healthcare trends, and evidence-based interventions within a multidisciplinary framework. Through the synergy of these professionals, patients experience holistic support that spans medical, lifestyle, and psychosocial dimensions, thereby enhancing their quality of life.
