Abstract
Introduction. Oesophageal atresia (OA) is a rare congenital anomaly resulting from abnormal development of the oesophagus. When primary anastomosis is not feasible, a cervical oesophagostomy may be created to divert salivary secretions; however, postoperative management can be challenging and may be associated with peristomal skin complications.
Methods. This case series describes the management of cervical oesophagostomy in two neonates with long-gap type A oesophageal atresia using a nursing-led adaptation of a one-piece, open-bottom enterostomy pouch. The device was introduced to improve secretion containment, protect the peristomal skin, and facilitate early oral stimulation.
Results. Following introduction of the enterostomy pouch, peristomal erythema improved and subsequently resolved, with no episodes of dermatitis, ulceration, maceration, or local infection observed during follow-up. The frequency of dressing changes was substantially reduced, supporting caregiver management at home. The pouching system also enabled early oral sensory stimulation while the infants awaited definitive oesophageal reconstruction.
Discussion and Conclusions. In these two cases, a one-piece, open-bottom enterostomy pouch provided a practical nursing strategy for cervical oesophagostomy management. The approach was associated with improved peristomal skin protection, fewer dressing changes, greater caregiver participation, and the feasibility of early oral stimulation. Further prospective evaluation is needed before broader conclusions can be drawn.
